What irregular & heavy periods involves

Periods that come unpredictably, last too long, or are heavy enough to cause anaemia are not something to live with. The causes range from PCOS and thyroid disease to polyps, fibroids, adenomyosis and, less commonly, endometrial pathology.

The investigation is quick and the treatment is usually straightforward once the cause is known. What matters is not simply suppressing the bleeding without asking why it is happening.

  • Cycles shorter than 21 or longer than 35 days
  • Bleeding for more than seven days, or flooding and clots
  • Bleeding between periods or after intercourse
  • Anaemia, fatigue or breathlessness from blood loss

Not everything on this list means you need this treatment. The workup comes first, and the recommendation follows from what it shows — not from what was booked before you arrived.

Step by step

  1. Assessment

    Cycle history, examination, haemoglobin, thyroid and prolactin, and a pelvic ultrasound.

  2. Cavity evaluation

    Saline sonography or hysteroscopy where a polyp or submucous fibroid is suspected.

  3. Medical treatment

    Tranexamic acid, hormonal treatment or a levonorgestrel IUS depending on the cause and your plans.

  4. Surgical treatment

    Hysteroscopic polypectomy or myomectomy, with ablation or hysterectomy only where appropriate.

When is heavy bleeding serious?

If you soak through protection hourly, pass large clots, or your haemoglobin is falling, it needs prompt attention.

Will I need a hysterectomy?

Rarely, and almost never as a first option in women who may want children. Most abnormal bleeding is controlled medically or by hysteroscopic surgery.
